The Carleton Place Regional Health Hub will provide the region’s communities with a comprehensive health services destination to help solve the high unfulfilled demand for primary care in the region. The Hub project applies the core principles of Ontario’s “Your Health Plan”- delivering the right care in the right place through convenient and connected primary care services. Anchored by a a full-floor (14,500 square feet) family clinic bringing together all 17 (and growing) local physicians and adding on-site administrators and nurse practitioners to provide efficiencies that will allow for substantial increases in patient attachments and visits, while reducing emergency department visits.

Effective patient-centered care occurs when doctors can easily access and collaborate with interprofessional health providers like pharmacy, diagnostic, specialists, therapists, and other primary care services. The Hub concept increases this efficiency by having allied health services locate in the same facility, helping create ease of contact, as well as continuity and comprehensiveness of patient care.
