TRANSFORMING THE DELIVERY OF PRIMARY CARE IN THE REGION
A purpose-built facility to support primary health care delivery according to best practices predicated by the Ontario Ministry of Health and Long-Term Care (MOHLTC): family physicians working with other family physicians, nurses, medical administrators, social workers, dieticians, and other health care professionals to deliver better access and continuity of primary care.
Supported by the Town of Carleton Place and the Township of Beckwith the facility will be anchored by a primary care medical clinic formed by local family physicians regrouping their existing practices. Extended primary care services and support will be ensured by the Ottawa Valley Family Health Team Carleton Place, with an office of on-site administrators, nurse practitioners, and other supporting providers.
